Athlete Eligibility and Medical Forms

Please Read First:

The department of QUEEN'S ATHLETICS has changed its method of approving eligibility for its athletes by adopting a web-based system for collecting required personal and athletic information. In addition, the Head Athletic Therapist has a similar web-based means for obtaining the information needed for medical and injury support for participants in our diverse sport program. Queen’s University Athletics Privacy Statement:

We ask you to read the following carefully.

In order for you to participate in sports at Queen’s, the ATHLETICS DEPARTMENT needs to collect, use, and retain certain personal information (and in some cases some medical information) from you as mandated by the Ontario University Athletics (OUA) and Canadian Interuniversity Sport (CIS).

The collection of personal and medical information from you is necessary in order to

  • administer the sporting activities you want to participate in
  • confirm your status as a full-time student
  • determine whether or not you are eligible to play in either, or both, leagues.
  • determine if you require special attention for medical reasons in order to allow a student-athlete to participate to his or her fullest extent as a varsity team, varsity club or competitive club member. The medical information is collected and used solely by the Head Therapist for this purpose.

While we limit our use and disclosure of your personal information to the above stated purposes, successful and award winning athletes may be asked for their consent to disclose their personal information to the media. We will not provide information we have collected on you to any third parties, other than those mentioned above, without your permission or unless required by law.

Athlete Eligibility & Medical Forms

There are links below to the 2 forms that MUST be completed by those wishing to compete as an Interuniversity Team/Club or Competitive Club athlete. A Medical Information Form for those athletes attending pre-season training camps MUST be filled out prior to the first day of camp. Athletes are advised to contact the Head Coach of those sports holding early or pre-season training camps.

Please take the time to read, and fill out, the two forms correctly. You will have an opportunity to review your personal and medical information during your team's respective eligibility night.

Eligibility Information Form

Athletes must contact and have approval from their Head Coach before filling out and submitting the eligibility form. The form must be filled out prior to Eligibility Night, and all forms MUST be submitted electronically. There will be NO exceptions to this request. An athlete will not be processed until he/she has filled out this form electronically. There will be facilities for completing the form electronically in the Physical Education Centre during the early Fall term. There are three steps to the eligibility process and ALL THREE STEPS must be completed before an athlete can be considered to be eligible to participate as a Queen's student-athlete.

The THREE STEPS are as follows:

  • Fill out the eligibility information form (includes an athlete profile)
  • Fill in the medical information form (this may need to be submitted earlier - see above)
  • Attend a drug awareness seminar and sign your corrected eligibility forms. A useful reference related to drug awareness for athletes is noted below. As in former years, the signing of your form includes acceptance of the Queen's student-athlete disclaimer as referenced below.

Although the medical form is straightforward to fill out the eligibility form needs a few explanatory comments. These comments can be broken into general ones and specific ones about individual questions.

General Comments

The form has been designed so that it cannot be submitted until certain fields have been filled in. These required fields are indicated by a red asterix. In some of these fields, default information has been provided to allow an incomplete form to be submitted for example, a fictitious local Kingston address and telephone number has been provided for those who do not know their Kingston address or telephone number when first filling out this form. IT IS THE RESPONSIBILITY OF THE ATHLETE to provide the correct information either electronically or in person (if changes are necessary), to Jeanean Sergeant (see below) before eligibility night. Specific Comments

SECTION I General Information

SPORT: Mandatory Field
Choose your sport from the pull-down menu. These are the only Interuniversity sports and varsity clubs offered by Queen’s Athletics & Recreation. The form will not allow for any sports other than what is listed currently. If you wish to register for more than one sport a SEPARATE ELIGIBILITY FORM MUST BE SUBMITTED for each additional sport or club you wish to register for.

STUDENT ID #: Mandatory Field
A default student number has been provided if you do not know yours, or have not been assigned a Queen's Student Number. If you know your Student ID number change the default number now.

DATE OF BIRTH: Mandatory Field
Fill in this field using month/day/year for example: 04/10/72.

HEIGHT: Mandatory Field
Fill in this field using either metric or Imperial units.

WEIGHT: This is required only for Football.
You can use either pounds or kilograms, but enter the units used.

LOCAL ADDRESS: Mandatory Field
A default address and telephone number has been placed in these fields. If you local address is known, please change the defaults. Any changes in the requested information should be reported to Manager, Inter-University Sport as soon as possible.

EMAIL ADDRESS
A default e-mail address is given. If you know your e-mail address change the default address. Any changes in your e-mail address must be reported to Manager, Inter-University Sport. Please use a Queen’s e-mail address if possible.

FACULTY/SCHOOL
Choose your faculty or school from the choices listed on the pull down menu. If you are unable to determine which faculty or school you belong to call Manager, Inter-University Sport for assistance.

ELIGIBILITY USED: Mandatory Field
This field refers to the years of O.U.A./C.I.S. eligibility used. DO NOT include this current year.

POSITION:
Not mandatory. Indicate position if known or if a team player during your last season.

SECTION II C.I.S. and O.U.A. Rules

You must read the brief section on C.I.S or O.U.A. Rules for Eligibility, then answer ALL OF THE QUESTIONS. The majority of the questions are pre-set with a default response. If the default response is not applicable to you then change the response. Fill out the required information in the fields having no default responses.

Question 8 clarification
In the case of Question 8, please indicate if you are receiving financial support, other than OSAP, administered through Queen’s University and/ or ii) an agency or scholarship outside of Queen’s University. All scholarships listed should have an amount each scholarship/bursary/academic prize etc. is worth.

The information provided through this section is used for media purposes. Fill in as much of the information as possible that applies to you.

All student-athletes MUST fill out these forms. Please take a few minutes to fill them out completely and accurately.
